How to Complete the Form?
Please read instructions below before completing the fillable PDF form.

How to Download and Complete the Form

– Tip: If you are viewing the PDF in your web browser and do not see a download option, press Ctrl + P (or Command + P on Mac) to open the print dialog. From there, choose Save as PDF and save the file to your desktop or device.
– Chrome: Check the bottom-left corner or the “Downloads” icon.
Safari / Firefox / Edge: Check your Downloads folder or top-right download icon.
3. Open the downloaded form using Adobe Acrobat Reader (recommended for proper form features).
5. Fill out all required fields.
6. Click Save to save your completed form to your device.
How to Email the Completed Form
1. Open your email (Gmail, Outlook, Yahoo, or your preferred email app) OR (click on the submit button on the form to open your preferred email app and click send).
2. Click Compose or New Email.
3. Click the Attach (paperclip) icon.
4. Select the saved, completed PDF form from your device.
5. Send the email to: info@fivestarbakeryent.com.
6. You will receive confirmation once we receive your form.
